2. that the contents of a document that you swore to or affirmed are true, 
3. that you personally appeared before us, 
4. that you were able to communicate with us, 
5. that you showed us your current, accurate and genuine identification, 
6. that you have read and understand the document and its purpose, 
7. that the document was written in English, 
8. that you filled in any blank spaces before signing, 
9. that the document or transaction did not commit any illegal or improper act or deception, 
10. that we did not provide any legal advice or legal review of your document, 
11. that we were not named as a party or beneficiary in the document, 
12. that we are not related to you, and are not business partners with you, 
13. that you were legally competent and authorized to sign, 
14. that you signed willingly, as your own free act and deed, and 
15. that we provided the type of notary service that you requested, as allowed by law.

 
16. You agree to carefully review our notarization certificate attached to your document and our notary journal entry, and to alert us of any problems before paying or leaving. 
17. You agree that your payment for our notary services or accepting any free notary services from us shall indicate your review, acceptance and approval of our notarization and notary journal entry. 
18. You understand that we are only liable for damages that are proximately caused by our notarial misconduct. 
19. You agree to take action to prevent, avoid, mitigate and minimize any damages caused by any error, omission or defect in our notarization. 
20. You agree to contact us and to alert all document recipients immediately if you become aware of any problems with the notarization or if the notarization is rejected. 
21. You agree to have your notarized documents reviewed by an attorney or to accept liability for any defects. 
22. You agree that, after three (3) days, you confirm and ratify that the notarization is fully accepted and approved by you. 
23. You understand that we reserve the right to refuse notary service requests that we believe are improper, unreasonable or not in compliance with any law, or transactions beyond our level of knowledge or experience.  

Colorado Medical Marijuana Registry: If we notarize an application for the Colorado Medical Marijuana Registry, you agree to release us from any liability related to the applicant's use of marijuana. Applicant agrees to only use marijuana for medical purposes, in limited amounts, as allowed by Colorado law, and to remain in clear and unambiguous compliance with state laws.  Applicant authorizes us to release any records we may have regarding medical marijuana registry application to law enforcement, drug enforcement and health department officials, and when required by court order.  

WARNING! THE USE, POSSESSION, DISTRIBUTION, AND MANUFACTURE OF MARIJUANA REMAINS A FEDERAL CRIME IN COLORADO, AND POSSESSION OF A REGISTRATION CARD PROVIDES NO PROTECTION WHATSOEVER AGAINST FEDERAL CRIMINAL PROSECUTION.  See Federal Department of Justice Guidance Letter, dated 10/19/2009, regarding states with laws authorizing medical use of marijuana.
